Once registration is complete, print off the required forms for clinical and obtain your physical, TB test, and fingerprint background check.
Program only accepts 10 students max at this time. IF the session is full, the Program Coordinator will convey that at time of registration and give the option to register for the next available session

Below are the listed forms you will need to participate in the clinical portion of the program. These are non-negotiable conditions of participation required by the clinical site and are a MUST in order to attend. With the exception of the web check authorization form, they must all be completed and returned to the Program Coordinator no later than the end of the first week of class. The health history is for you to fill out simply listing all of your medical background. The physical form you print and take with you for a physical or you can obtain a recent copy of a health physical from your family physician. The TB test form needs to be printed and taken to your physician or local health department for completion of this test. Only a ONE step is necessary for our program. The student WEB check Authorization form is to be printed out and TAKEN with you to one of the locations listed on the form. You MUST fill out the top portion, check the box marked BCI. You will hand this form to the person at the ESC and they will bill me for your background check.
